Recurring Direct Debit Payments Authorization

Your authorization is required in order for M&T Bank to automatically transfer funds to make payments on a recurring basis. In order for M&T to commence your recurring payments, you are required to complete and sign an authorization and mail it to the appropriate M&T address indicated at the bottom of the form. Please retain a copy of the authorization for your records.
